We are seeking a full-time Nurse Practitioner or Physician Assistant to join our community-focused outpatient family medicine clinic in Dewey-Humboldt, AZ . Key Responsibilities
Perform annual wellness exams, preventive screenings, and Medicare visitsDiagnose and manage acute conditions and minor injuriesProvide chronic disease management (diabetes, hypertension, COPD, etc.)Order, interpret, and follow up on diagnostic tests and imagingEducate patients on medications, treatment plans, and healthy lifestyle practicesMaintain accurate EHR documentation with synchronous scribe supportCoordinate referrals and manage transitions of careParticipate in clinical quality initiatives and practice improvement effortsQualifications
Active and unrestricted Arizona NP or PA licenseCurrent DEA registration or ability to obtainCredentialing eligibility with Medicare, Medicaid, and commercial insurersNo history of malpractice or disciplinary actionGraduate of an accredited U.S.-based NP or PA program(Note : Puerto Rico-based programs are not accepted)Who Thrives Here
