Overview

We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our client's growing team in Grantville, Pennsylvania. This role focuses on providing exceptional, patient-centered nursing care within the comfort of patients' homes. The ideal candidate will demonstrate strong clinical judgment, excellent communication, and a passion for helping patients achieve optimal health outcomes in a supportive home environment.

Key Responsibilities

Provide skilled nursing care and conduct comprehensive assessments for patients in their homes

Develop and implement individualized care plans in collaboration with physicians and the healthcare team

Educate patients and families on health management, medications, and treatment plans

Monitor patient progress and adjust care plans as needed to ensure positive outcomes

Maintain accurate, timely, and compliant documentation in accordance with agency and regulatory standards

Coordinate with interdisciplinary team members to deliver holistic, quality care

Schedule and Shift Details

Monday through Friday, 8 : 00am 5 : 00pm

On-call weekend rotation every 4 8 weeks (Saturday & Sunday, 8 : 00am 5 : 00pm)

Flexible scheduling and limited weekend work

This is a Pay Per Visit position

Travel

Local travel within the Grantville area required (travel reimbursement provided)

Qualifications

Required

Associate degree or higher in Nursing

Active Registered Nurse (RN) license in Pennsylvania

Minimum of 1 year of professional RN experience (home health preferred)

Current BLS or ACLS certification through the American Heart Association (training assistance available if needed)

Valid driver's license, reliable vehicle, and current auto insurance

Preferred

Prior experience in home health or community-based nursing

Strong assessment, organizational, and interpersonal skills
