Over the last 25 years, Your Elder Experts has grown into the largest not-for-profit Aging Life Care program in the Greater Boston Area. Our talented and caring professionals guide older adults and their families through the complexities and challenges of aging well. We have expertise in working with people with dementia, Parkinson's Disease, ALS, and other chronic illnesses, though we work with people in all situations. We take pride in welcoming clients from the LGBTQ+ community. And we are especially attuned to the needs of people aging without given or chosen family to support them ("solo agers").

Whether through one-time consultation or ongoing care management, we help our clients in myriad ways : exploring housing options, understanding their benefits and insurance, learning about new programs and resources, communicating effectively with family members around difficult issues, and coordinating medical services, home care, and community supports. We also accompany our clients to medical appointments to provide accurate history, communicate current issues and make sure that treatment plans are followed-up on.

ABOUT THE ROLE

The Aging Life Care Manager is a role for an experienced elder care professional looking to join a trusted organization where older adults and their families get the customized services they need. Your workday will include :

• Evaluating the strengths and needs of older adults and adults living with chronic illnesses.
• Educating clients and their families / care partners about options and resources
• Creating and managing client-centered plans of care and adapting plans as needed
• Facilitating client and family / care team meetings and relationships
• Building relationships with community service providers and elder-care professionals
• Documenting care plans and care management
• Management and advocacy for appropriate and ongoing medical care.
