Registered Dietitian

Job Duties :

• Coordinate nutritional care of residents and patients by completing nutritional assessments.
• Participate in care planning meetings as required.
• Provide nutritional counseling and education for residents and family.
• Completes comprehensive assessments on patients as required.
• Provides dietary in-service training as required.
• Under PDPM, identify, document, and treat nutrition related diseases for improved outcomes.
• Meet DOH / state / federal and company standards.
• Perform duties independently without training or direction.
• Perform other duties as assigned by management.

Required Skills / Abilities :

• Good decision making and judgment capabilities.
• Ability to identify and solve complex problems.
• Great communication and interpersonal skills are a must.
• High comfort level with enteral nutrition, wounds, weight management, Hemodialysis.
• Comfortable working in a healthcare facility.
• Familiarity with state survey process and compliance with nutrition care regulations.

Education and Experience :

• Bachelor’s degree in nutrition, health, or related field.
• Must possess Registered Dietitian licensure within the state of operations
• At least one year of experience in a long-term care setting
• Experience with MDS 3.0.
• Experience with electronic charting such as Point Click Care is a plus

Physical Requirements :

• Prolonged periods standing and walking.
• Must be able to lift up to 25 pounds at times.
