Location : Long Island, NY (local travel required)

Position Overview :
We are seeking a compassionate and experienced Elder Care Coordinator (ECC) — a Social Worker, Registered Nurse, or Geriatric Care Professional — to join our growing Life Care Planning team. This is a unique opportunity to make a meaningful difference in the lives of older adults and their families while working within a supportive, mission-driven private law firm.
Key Responsibilities :
- Provide case management, advocacy, and care coordination for elderly clients and their families.
- Conduct client assessments and develop care plans that align with medical, financial, and legal goals.
- Offer crisis intervention and ongoing support to clients and caregivers navigating long-term care challenges.
- Collaborate closely with our attorneys to ensure clients receive optimal legal and care outcomes.
- Build and maintain relationships with community partners, medical professionals, and referral sources.
- Represent the firm at professional and community events to increase brand awareness and engagement.
- Perform local travel to clients’ homes, facilities, and events (some after-hours / weekend participation required).
Qualifications :
MSW , RN , or 5+ years of professional experience in geriatrics, care management, or long-term care.Deep understanding of the aging process, long-term care systems, and community resources.Strong interpersonal, organizational, and communication skills.A genuine passion for helping seniors and families navigate complex care needs.Reliable transportation and willingness to travel locally.Why Join Tully Law Group :
