Job Summary :
As the Treatment Plan Nurse for the BHU (Behavioral Health Unit) at Mission Community Hospital, you will play a crucial role in providing individualized and holistic treatment plans for patients suffering from mental health and psychiatric disorders. This is a full-time, individual contributor role that requires strong clinical knowledge, excellent communication and interdisciplinary skills, and a passion for helping patients achieve optimal mental health. The job is located in Panorama City, California.

Responsibilities :
Conduct comprehensive psychiatric and biopsychosocial assessments of patients upon admission to the BHUDevelop individualized treatment plans for patients in collaboration with the interdisciplinary treatment teamMonitor and evaluate patient progress towards treatment goals on a regular basisCommunicate with patients and their families to educate them on treatment plans and provide supportCoordinate and facilitate interdisciplinary team meetings to discuss patient progress and make necessary adjustments to treatment plansAssist in the delivery of evidence-based therapy and interventions as neededDocument all patient interactions and progress in electronic medical recordsCollaborate with community resources to ensure seamless discharge planning and continuity of care for patientsAdhere to all hospital policies, procedures, and standards of careParticipate in ongoing quality improvement initiatives for the BHURequirements :
Active RN license in the state of CaliforniaBachelor's or Master's degree in Nursing from an accredited institutionMinimum of 3 years of experience in a psychiatric or behavioral health settingStrong knowledge of mental health disorders, treatments, and medicationsExcellent assessment, critical thinking, and problem-solving skillsAbility to work effectively as part of an interdisciplinary teamStrong communication and interpersonal skillsExperience with electronic medical recordsCPR certificationBilingual (English / Spanish) preferred but not requiredEEOC Statement :
