This position focuses on post-partum, neonatal, and maternal health assessments, patient education, and coordination of care plans. Nurses will conduct 4–8 home visits per day and must have solid experience in Labor & Delivery and NICU care. Duties : Provide home-based nursing care to maternal and pediatric patients. Conduct comprehensive assessments and deliver individualized care plans. Educate patients and families on post-partum and infant care. Collaborate with interdisciplinary teams to ensure quality and continuity of care. Maintain accurate clinical documentation. Required Skills & Experience : Minimum of 2 years of acute care experience in Med-Surg, Maternal and Child Care, Critical Care, or Pediatrics. Current Maternal Child Home Health experience. Education : Required : Associate’s Degree in Nursing (ASN, ADN) from an accredited program. Certifications & Licensure : NRP Certification Current New York State RN License Primary Source Verification from NYSED AHA BLS Certification Important Notes Must have experience in L&D and NICU. Rotating weekends required.
