We are a mobile mental health practice in Baltimore servicing clients in assisted living facilities and daycare centers. Seeking a nurse practitioner to join our growing family! This position will be delivering care to patients in Baltimore County and City that are home-bound or in the community via in person or remote.

We bring mental health right to where our patients call home. The nurse practitioner will provide comprehensive care to patients diagnosing and treating acute or chronic mental health conditions.

POSITION SUMMARY : The PMHNP will provide comprehensive psychiatric care to a variety of patients in the community and coordinate health services between the Agency, community and home settings. You will be responsible for conducting psychiatric assessments, assessing physical health needs, coordinating all somatic referrals to necessary specialists, and assuring lab workups are conducted as needed. The NP is responsible for the overall clinical service delivery of the team and will conduct a medical assessment, psychiatric evaluation, medication assessment and management, and monitoring of the overall somatic and mental health of clients.

Nurse Practitioner / NP Job Education and Experience Requirements :

• Active State NP license
• ANCC board-certified as PMHNP
• Current BLS certification
• Active CDS /
• DEA
• Experience as a Psych NP
• Prior Home Health Nursing / house call experience, a plus but not required.
• Licensed PMHNP in the State of Maryland.
• Must have a valid drivers license, clean driving record and means of reliable transportation.
