Work Hours : Monday to Friday, 9 : 00am to 6 : 00pm OR Monday, Wednesday, Thursday, Friday, 7 : 00am to 6 : 00pm
Responsibilities :
- Evaluate and assess patients' physical therapy needs, formulate specific treatment plans and goals, and implement these plans with physician approval.
- Completion of all required documentation in an accurate and timely manner :
Initial evaluations
Daily treatment notesStaffing reportsDischarge summariesTimely reassessmentEducational activities for patientBilling-related documentationInsurance justification letters for equipment purchase and rentalsOther patient care-related documents as neededProvide patient educational services, both verbally and in writing, documenting all educational activities and patient responses in charts.Supervise the administration of physical therapy services by the Physical Therapy Assistant as needed.Participate in treatment plan reviews, patient case review conferences, patient assessments and reassessments, and staffing with physicians.Adhere to the company's established policies and procedures, including strict patient confidentiality.Assist the Medical Director as directed in the following, but not limited to : Utilization Review & Quality AssurancePerform other duties as assigned.Requirements :
Bachelor's, Master's or Doctorate in Physical Therapy from an accredited institution.Must have a license (in good standing) or diploma to practice physical therapy from the State of Arizona.Current CPR Certification.Benefits :
