- Working in the town of New London, CT
- 15 -20 patients per day
- Full-time
- Nurse practitioners (NP) and physician assistants (PA) are welcome to apply
- Compensation and Benefits : Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated income range of $134,000 to $158,000 annually with no cap on productivity bonus income potential. This full-time role is benefit eligible.
Responsibilities
Drive high-quality, safe, and cost-effective patient care in post-acute care environmentInpatient rounding in skilled nursing facility, inpatient rehabilitation facility, and long-term care hospitalCollaborate with other clinician interdisciplinary teams and follow CMS guidelinesWork closely with hospice, palliative care, and home care agencies for safe patient transitionsEnsure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelinesQualifications
Board certified FNP, AGACNP, ACNP, AGPCNP, or PA-CWilling to train experienced candidates as wellLicensed in stateCalifornia Applicant Privacy Act : -applicant-privacy-notice /
