You will coordinate all facets of the patient's at-home care, collaborating with medical professionals and delivering compassionate care to our patients. Your responsibilities will include : \n

• Conducting initial visits to assess patients' needs and determine eligibility for home care services.\n
• Developing personalized care plans tailored to each patient.\n
• Ensuring the highest quality of care is maintained throughout the patient's home health journey.\n\nQualifications : \n
• Graduated from an accredited diploma, Associate, or Baccalaureate School of Nursing.\n
• Possess an active Registered Nurse license in your state.\n
• Hold a valid state Driver's License.\n
• Have at least one year of experience as a Registered Nurse (two years preferred).\n
• Provide proof of current CPR and Hepatitis consent / declination.\n
