About The Position
Medix is working with a client facility to secure a skilled Licensed Master Social Worker (LMSW) for a rewarding travel assignment starting in early November. This is an incredible opportunity to be part of a brand-new facility providing PACE (Program of All-Inclusive Care for the Elderly) services, focused on helping seniors remain active and safely in their community located in Jacksonville, IN. The PACE center operates like a day center for participants who are typically active and mobile.
As the center is new, you will have a unique chance to help shape the program as the census grows, with a focus on comprehensive social work services for participants aged 55 and older.
This facility is part of a rapidly expanding organization, offering excellent long-term possibilities for travelers who enjoy their contract.

Facility Type : Outpatient (PACE Center)
Start Date : 11 / 3 / 2025
Length of Assignment : 13 Weeks (Option for 6 months)
Shift / Hours : Monday - Friday : 8AM-4 : 30PM, covering five 8 hour shifts. No on call, weekends, evenings, etc.
State License Needed : Indiana (or reciprocity)
Pay : Compensation packages, including hourly rates and possible stipends, are highly competitive and will be discussed in detail with your dedicated Medix recruiter.
Role and Responsibilities :
The Social Worker will provide essential social work services as part of the Interdisciplinary Team (IDT), including advocating, assessing, assisting, collaborating, educating, evaluating, implementing, and planning for participants.
Conduct intake assessments, biopsychosocial assessments, and cognitive / emotional assessments (e.g., GDS, SPMSQ) to evaluate psychosocial functioning.
Provide ongoing social work services to help participants and their representatives address personal and environmental challenges that may interfere with their health or benefit from the PACE program.
Coordinate care with the RN Case Manager and the broader IDT, ensuring social work-related concerns are documented and interventions are applied.
Facilitate outside referrals and educate participants on using community, health, and public services / benefits (e.g., SNAP, Medicaid, Medicare).
Provide support for participants and caregivers, including crisis intervention, adjustment to illness / disability / treatment, and end-of-life care planning.
Ensure timely and appropriate communication during / post hospitalization and assist with discharge planning.
Documentation will be completed using the IntusCare (PACE specific) EHR.
Daily patient volume will be low to start in this new center and will increase gradually.

Job Requirements
One (1) year of experience working in a healthcare setting with a frail or elderly population is required.
Ability to organize and manage a workload to ensure the provision of appropriate and maximized social services.
