Description :

The Social Worker will serve as a care coordinator for discharge planning.

This role involves coordinating discharge for complex patients across various inpatient units.

The Social Worker will maintain a caseload of 15-20 patients per day.

The work will include assessing discharge readiness and facilitating appropriate placements.

Collaboration with other healthcare providers will be essential in this position.

The Social Worker must adhere to established criteria for patient placements.

Documentation will be done using the EPIC charting system.

Consultation with relevant departments will occur as needed.

The work requires flexibility and adaptability to address complex patient needs.

A strong understanding of discharge planning processes is necessary.

The Social Worker will report to the Social Work Manager.

No time off will be approved the week before holidays, and holiday scheduling will be required as needed.

The position will involve working alongside a diverse team of social workers.
