Skilled Nursing Facility experience is preferred.

COVID test required 72 hours prior to start date with results issued 48 hours prior to start date.

Description :

All candidates are required to undergo a thorough credentialing checklist prior to submission.

This includes verification of state licenses, resumes, and references from three individuals.

Background checks will include a county criminal check for the last seven years for all counties lived and worked in.

Additionally, a federal criminal check, a multi-state criminal search, and a nationwide sex offender search must be completed for the previous seven years or more.

A search through the HHS / OIG and GSA databases is also required.

Social Security Number trace is necessary.

A single step PPD or Quantiferon test, or proof of a positive PPD and a chest X-ray is mandatory.

This position is of normal urgency.
