Overview :
This role is rooted in traditional, high-touch care managementyoull be in office, remotely helping patients navigate their health journeys. Youll also be using our novel AI-powered platform every day to manage your work, and youll play an active role in shaping how these tools evolve. Your insights from direct patient care will feed directly into the design of future technology, giving you a chance to influence not only individual patient outcomes but also the broader direction of tech-enabled care delivery.
Responsibilities :
- Onboard patients into the Advanced Primary Care Model (APCM) through proactive outreach, building trust, and setting them up for long-term success, leveraging state-of-the-art AI tools.
- Design and validate personalized care plans in Healthy, working hand-in-hand with patients and physicians to align on goals and next steps.
- Engage and re-engage patients who need extra support, ensuring no one falls through the cracks.
- Coordinate care services recognized by CMSfrom transportation and medication management to addressing social needsso patients get the right help at the right time.
- Track and document patient interactions using Healthy and practice EMRs, ensuring accuracy, compliance, and seamless communication across the care team.
Qualifications :
Bilingual : Spanish fluency is highly preferredLicensed Practical Nurse (LPN) license preferred, though Certified Medical Assistants or other strong clinical backgrounds will be considered must be eligible for licensure in Florida, if applicablePrior experience as a Care Manager, or similar patient-facing roleComfortable working in and across multiple EMR systemsPreferred Skills :
Experience working with value-based care programs (APCM, CCM, TCM)Familiarity with CMS billing requirements or care management documentationInterest in growing a career in AI
