PHYSICAL THERAPIST HOME HEALTH – PART-TIME (South Hills, Pittsburgh, PA)

Due to strong community support, Bridges Therapy Associates is seeking a Part-Time Home Health Physical Therapist to join our team. The qualified candidate will provide effective, patient-centered services that will promote functional improvement and maximize patient potential in their home environment. The successful candidate will have a professional work ethic and a desire to make a positive difference in the lives of their patients.

RESPONSIBILITIES :

• Perform routine home physical therapy visits including evaluations, starts of care, daily visits, reevaluations, discharges
• Provide high quality patient care, evaluates functional needs and outcomes, and deliver effective and patient-centered treatments to achieve desired outcomes
• Designs and implements an appropriate plan of care for patients based on a thorough assessment of their functional needs in their home environment
• Conducts Physical Therapy practice within defined standards of care
• Communicates effectively with interdisciplinary team and support staff to achieve positive patient outcomes
• Work with and communicate effectively with PTA staff as appropriate
• Perform and document appropriate visits in timely manner
