Position Summary :
The Dietitian, under the supervision of the PACE Program Manager, is responsible for planning and providing nutritional care to PACE participants as part of an Interdisciplinary Team. This role includes assessing nutritional needs, developing and implementing individualized care plans, evaluating outcomes, and documenting results. The Dietitian ensures care is provided in accordance with professional standards, regulatory requirements, and the mission of the PACE program.
Key Responsibilities :
- Maintain knowledge of PACE Federal, State, and local regulations, the Compliance Accountability Program, and the Code of Ethics, ensuring adherence to professional standards.
- Obtain and evaluate nutritional / medical histories, lab data, test results, medications, and diet orders to determine participant needs.
- Recommend and implement nutrition interventions to maintain or improve health outcomes.
- Complete nutrition assessments per regulatory guidelines and as needed to support care planning.
- Educate participants, families, staff, and contracted providers on diet, nutrition, food safety, and sanitation.
- Document participant information and care plans in the electronic medical record (EMR).
- Participate in daily Interdisciplinary Team meetings and scheduled team activities.
- Perform additional duties as assigned.
Qualifications : Education :
Bachelor of Science degree in Dietetics and / or Food & Nutrition that meets or exceeds requirements of the American Dietetic Association for the registration examination.Successful completion of internship or coordinated undergraduate program per American Dietetic Association guidelines.Licensure / Certifications :
Registered or registration-eligible with the Commission on Dietetic Registration.Licensed or license-eligible with the State of Iowa.Current CPR / BCLS certification.Maintains Adult Mandatory Reporter status.Food protection / safety certification (ANSI-accredited, e.g., ServSafe) or ability to obtain within 90 days of hire.Valid driver’s license and auto insurance.Required Skills / Abilities :
Effective oral and written communication skills to engage with participants, families, and healthcare professionals.Ability to collaborate effectively within an Interdisciplinary Team.Strong organizational and time-management skills.Flexibility to manage shifting program priorities.Proficiency in Microsoft Office products.Certification Requirements
