POSITION SUMMARY :

The Therapeutic Care Manager is responsible for providing respite, advocacy, developmental life skills training, and mentoring services to children and adolescents in Hillsborough County, within both home and school environments, as part of Gracepoint's Community Action Team.. In addition, will provide coordination and continuity of evidence-based care for children / adolescents and families in need through a collaborative process of triage, assessment, planning, and communication in their homes and the community. Participates as a member of a community-based multidisciplinary treatment team. The Incumbent will report to the unit / department supervisor.

QUALIFICATIONS :

WORK ENVIRONMENT :

Requires traveling within Hillsborough County via personal vehicle to facilitate client services in the home and school in a variety of socioeconomic neighborhoods. High-paced, collaborative team performance with an emphasis on critical thinking and decision-making with guidance from program supervisor / manager. Climate-controlled office environments with frequent exposure to clients.

PHYSICAL DEMANDS :

Abilities in reaching, bending, talking, sitting, carrying, standing, grasping, fine hand coordination, hearing, reading and writing, and remaining calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. May be required to complete standing presentations.
