Physical Therapist – Home Health

Location : Waterford, CT

Schedule : Full-time

Setting : Home Health (training provided)

Overview :

We’re hiring a Physical Therapist to join a dedicated home health team serving patients in Waterford, CT and surrounding communities. Whether you're experienced in home health or looking to transition into this setting, training will be provided to set you up for success.

This is a rewarding opportunity to provide one-on-one care in patients' homes, helping them improve mobility, safety, and independence — all on their own terms.

Responsibilities :

Deliver physical therapy services in patient homes

Perform evaluations and develop individualized care plans

Coordinate with nurses, aides, and other therapists as part of an interdisciplinary team

Focus on functional progress, safety, and patient education

Maintain accurate documentation in accordance with state and agency standards

Qualifications :

Active Connecticut Physical Therapist license

Minimum 1 year of clinical experience (home health experience a plus, but not required)

Strong communication and time-management skills

Comfortable working independently in a field-based role

Must have a valid driver's license and reliable transportation
