Overview :

Under the general supervision of the Physical Medicine Supervisor and according to standards of treatment, performs objective assessments as well as comprehensive individualized treatment programs for patients of all ages according to departmental policies and procedures regarding referral.

  • Perform initial assessment on each patient upon receiving referral. Document all information according to departmental policies and procedures.
  • Administer appropriate modalities, exercise regimens, or treatment procedures according to departmental policies and procedures on standards of treatment care.
  • Participate in promoting education within the department.
  • Perform miscellaneous job-related duties as assigned by the Inpatient Physical Medicine Supervisor.
  • Demonstrates knowledge of special needs and behaviors of specific age groups to include neonatal, pediatric, adolescent, adult and geriatric.

Job Description :

Perform initial assessment on each patient upon receiving referral. Document all information according to departmental policies and procedures.

1.1 Initiate initial treatment visit according to departmental standards.

1.2 Inquire as to patient's perceived goals of treatment of Speech Pathology.

1.3 Inform the patient of modes of achieving perceived goals courteously.

1.4 Perform appropriate objective tests according to departmental policies and procedures regarding standards of treatment.

1.5 Document Initial Evaluation / Plan of Care according to departmental policy.

Administer appropriate modalities, exercise regimens, or treatment procedures according to departmental policies and procedures on standards of treatment care.

2.1 Perform individualized treatment procedures in a professional and courteous manner.

2.2 Reassess and update goals at least every 2 weeks.

2.3 Utilize appropriate safety techniques and infection control methods according to departmental policies and procedures.

2.4 Courteously explain treatment procedures prior to administering to patient.

2.5 Recommend equipment prior to discharge.

2.6 Collaborate with other professionals as appropriate for the treatment of all interdisciplinary patients as described in the respective plan of care and according to departmental standards of care.

2.7 Identify and provide written home instructions, outlining repetitions, frequency, and precautions 24 hours prior to discharge or completion of treatment sessions.

2.8 Educate family member or caregiver in rehab performance as well as patient's home procedures and precautions in a professional and courteous manner.

2.9 Document daily progress notes clearly with patient's responses to treatment procedures and consideration to pre-established goals according to documentation standards and guidelines.

2.10 Document a daily treatment record of each patient;record itemized patient charges for services and equipment on a daily basis with 95% accuracy.

Participate in promoting education within the department

3.1 Attend 80% of departmental inservice meetings / staff meetings and 100% of all mandatory hospital meetings.

3.2 Participate in required Quality activities as needed for the department and hospital.

3.3 Accrue appropriate number of continuing education credits each monitoring period according to the requirements of supervising and licensing agencies.

3.4 Maintain current CPR license.

3.5 Supervise Speech-Language Pathology students according to Mercy Health Center and School policies and procedures.

3.6 Assist in training new co workers as needed.

Perform miscellaneous job-related duties as assigned by the Inpatient Physical Medicine Supervisor.

4.1 Communicate to physicians with professional language and in a courteous manner via telephone, written correspondence, or personal contact.

4.2 Direct aides and rehabilitation technicians in safe and timely performance of transportation of patients, cleaning duties, or low level patient care to allow improved and efficient performance of the Physical Medicine and Rehabilitation Department.
