Access Health Louisiana, a network of Federally Qualified Health Centers (FQHCs) providing quality, affordable, and convenient care in high-need areas, is seeking a dedicated Licensed Clinical Social Worker or Licensed Professional Counselor to join our team. We are committed to delivering compassionate healthcare and ensuring that no patient is turned away due to their ability to pay.
Responsibilities :
- Conduct comprehensive assessments and provide individual, group, and family therapy.
- Collaborate with psychiatrists, primary care providers, and referral sources to coordinate holistic care.
- Maintain accurate and timely case documentation, updating records as needed.
- Actively contribute to a multidisciplinary team focused on improving health outcomes.
