Join us at our Bariatric Surgery Department, where you will play a vital role in patient care! We are seeking a dedicated Bariatric Nurse Navigator to enhance patient experiences and contribute to their health journeys.

About the Role :

As a Bariatric Nurse Navigator, you will be instrumental in providing direct and comprehensive patient care, from initial assessments to ongoing support. Working under the guidance of physicians and advanced practice providers, you will assist in various aspects of patient management and care coordination.

Key Responsibilities :

  • Measure and record vital signs accurately; promptly report any abnormalities to the healthcare provider.
  • Collect and document comprehensive medical, surgical, and social histories, along with focused physical assessments related to clinic visits.
  • Administer medications and treatments in accordance with physician or APP directives.
  • Support in-office procedures relevant to the specialty, ensuring thorough documentation in the electronic health record (EHR).
  • Evaluate and manage incoming test results, patient requests, and pharmacy renewals, facilitating effective communication with patients as needed.
  • Utilize resources effectively to achieve optimal patient outcomes, contributing to quality metrics at the facility, state, and national levels.
  • Delegate appropriate tasks based on the licensure of LPNs and MAs.
  • Maintain adherence to regulatory standards and compliance policies throughout operations.
  • Assist in the orientation and education of newly hired staff members.
  • Engage in continuous education to stay updated on current clinical practices.
  • Deliver education to patients regarding their conditions and health maintenance.

Minimum Qualifications :

  • Associate's degree from an accredited Registered Nursing program is required.
  • NYS current RN licensure is mandatory.
  • Current CPR certification is required.
  • Specialty certification in an ambulatory setting is preferred.
  • Exceptional customer service and communication skills are essential.
  • Ability to present information clearly and respond effectively to questions from patients, providers, and colleagues.
  • Proficient in MS Word and Excel, with a strong comfort level using computer software.
  • Experience with electronic health records is preferred.
  • Capable of administering medications via all routes and skilled in calculating medication doses.
