Case Manager - Rehab

The RN Case Manager - Rehab identifies adult and pediatric rehab candidates who meet program criteria and who will benefit from program services by completing a thorough assessment of candidates including- chart review, clinical assessment, and interaction with the ancillary staff and family directly involved in the candidate s care. The RN Case Manager - Rehab is responsible for collecting, comparing, collating, entering, and processing the information required for facilitating payment of patient stays on Rehabilitation. This individual serves as part of the collaborative care team, addressing the patient s plan of care and promoting patient-centered outcomes that will encourage the highest possible level of independence and quality of life. The RN Case Manager - Rehab serves as a resource to the care team with regards to discharge planning and provision of quality and cost-effective health and social care services.
