Description : Position Summary :

The Program Specialist supports the design, coordination, and implementation of day-to-day services for individuals with disabilities in a site-based setting. This role is responsible for ensuring person-centered programming, promoting independence, and supporting clients in achieving their individual goals. The Program Specialist collaborates with staff, families, and external partners to maintain a safe, supportive, and enriching environment.

Key Responsibilities :

• Develop and implement individualized service plans in coordination with clients, caregivers, and interdisciplinary teams.
• Facilitate skill-building activities, group sessions, and structured programs focused on independence, communication, community integration, and health.
• Monitor and document client progress toward goals; update plans as needed based on performance and feedback.
• Ensure all activities are delivered in alignment with licensing standards, organizational policies, and safety protocols.
• Provide direct support to clients as needed, including assistance with mobility, self-care, or behavioral support in accordance with individual support plans.
• Maintain accurate, timely, and confidential client records and reports.
• Collaborate with case managers, therapists, job coaches, and support staff to ensure coordinated care.
• Support training and onboarding of new staff members and contribute to a positive team environment.
• Participate in team meetings, interdisciplinary reviews, and professional development activities.
• Assist with the planning and execution of site events, outings, and community partnerships.
