Description : Position Summary :
The Program Specialist supports the design, coordination, and implementation of day-to-day services for individuals with disabilities in a site-based setting. This role is responsible for ensuring person-centered programming, promoting independence, and supporting clients in achieving their individual goals. The Program Specialist collaborates with staff, families, and external partners to maintain a safe, supportive, and enriching environment.
Key Responsibilities :
- Develop and implement individualized service plans in coordination with clients, caregivers, and interdisciplinary teams.
- Facilitate skill-building activities, group sessions, and structured programs focused on independence, communication, community integration, and health.
- Monitor and document client progress toward goals; update plans as needed based on performance and feedback.
- Ensure all activities are delivered in alignment with licensing standards, organizational policies, and safety protocols.
- Provide direct support to clients as needed, including assistance with mobility, self-care, or behavioral support in accordance with individual support plans.
- Maintain accurate, timely, and confidential client records and reports.
- Collaborate with case managers, therapists, job coaches, and support staff to ensure coordinated care.
- Support training and onboarding of new staff members and contribute to a positive team environment.
- Participate in team meetings, interdisciplinary reviews, and professional development activities.
- Assist with the planning and execution of site events, outings, and community partnerships.
Requirements :
High school diploma or equivalent required; associate’s or bachelor’s degree in human services, education, psychology, or a related field preferred.Minimum of 1–2 years of experience working with individuals with disabilities or in a similar support role.Strong communication, organization, and interpersonal skills.Ability to handle crises calmly and professionally.Basic computer skills for documentation and reporting.Must pass background checks and any required certifications (e.g., CPR / First Aid, crisis intervention).Physical ability to assist clients as needed, including lifting or supporting mobility aids.Preferred Attributes :
Knowledge of person-centered planning and disability rights.Bilingual (Spanish / English or ASL) a plus.Familiarity with Medicaid waiver programs, DARS / TWC, or local disability service systems.
