• Competitive salary range - $75,000–$83,000 annually, with room for flexibility for the right candidate\n

• Conduct comprehensive assessments of patients' physical, emotional, psychosocial, and spiritual needs to develop individualized care plans.\n
• Coordinate interdisciplinary care team meetings to ensure collaboration among healthcare professionals involved in the patient's care, including physicians, social workers, chaplains, and volunteers.\n
• Educate patients and their families about disease progression, symptom management, medication management, and end-of-life care options to empower them to make informed decisions.\n
• Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes in compliance with regulatory requirements and organizational policies.\n
• Provide compassionate end-of-life care, including symptom management, comfort measures, and support for patients and their families during the dying process.\n
• Participate in quality improvement initiatives aimed at enhancing the quality of care provided to hospice patients and their families.\n\nA few must-haves for our Registered Nurse (RN) Case Manager : \n
• Graduate of an accredited nursing program and active RN license in Louisiana\n
• Minimum of 1 year of hospice nursing experience\n
• CPR certification\n
• Valid driver's license, auto insurance and reliable transportation\n
• Knowledge of state and federal home health regulations\n
