LPN Nursing Coordinator Part-Time (Immediate Opening)

Mishawaka, IN | Part-Time | $26$30 / hr (based on experience, $30 / hr cap) | 3 days / week Flexible schedule | Hybrid (field + remote)

Were seeking an experienced and organized Licensed Practical Nurse (LPN) to serve as our LPNNursing Coordinator. This role is ideal for a nurse who loves case management, training caregivers, and maintaining compliance without heavy bedside nursing or overnight shifts.

Key Responsibilities

  • Complete initial and ongoing client assessments, update care plans, and maintain EVV-compliant documentation.
  • Manage Integrated Health Care Coordination (IHCC) cases wellness checks, medication reminders, communication with physicians and case managers.
  • Provide in-home caregiver training, shadowing, and binder audits to ensure quality and compliance.
  • Review and approve caregiver visit documentation to meet state & Medicaid waiver standards.
  • Support the Director with quality checks, audit prep, and clinical oversight.

    • Hybrid Schedule

  • 3 flexible 8-hour days per week (MonFri) choose days based on client need.
  • If preferred, you may schedule one of your days on a weekend to complete client assessments or intakes.
  • 11.5 days in the field (assessments, binder audits, caregiver support).
  • 1.52 days remote (documentation, case management, care coordination).

    • Ideal Candidate

  • Active LPN license in Indiana (required).
  • Experience in home care, long-term care, or Medicaid waiver programs preferred.
  • Strong skills in documentation & compliance (AxisCare experience a plus).
  • Enjoys mentoring caregivers and improving care quality.
  • Self-directed, detail-oriented, and dependable.
