Job Summary :

This position focuses on delivering brief, evidence-based therapy to adult patients as part of an integrated care team. Working alongside medical professionals, you'll address a range of mental health needs through short-term interventions designed to improve patient well-being within a primary care setting.

Schedule : Monday to Friday | 8 : 00 AM – 5 : 00 PM

Salary Range : $81,000 – $90,000, commensurate with experience

Key Responsibilities :

• Conduct comprehensive assessments and deliver brief therapy using evidence-based approaches such as behavioral activation and problem-solving therapy.
• Support primary care providers with mental health consultations and collaborate on patient care plans.
• Screen adults and older adults for behavioral health needs, including substance use, trauma, and co-occurring disorders.
• Provide referrals and linkages to appropriate community services when necessary.
• Participate in interdisciplinary team meetings, case reviews, and care coordination.
• Facilitate therapeutic or psychoeducational groups targeting mental and chronic health conditions (e.g., depression, diabetes).
• Ensure timely and accurate documentation in the electronic health record.
• Maintain compliance with state / federal laws and ethical standards for clinical care.
