Pyramid Consulting is seeking a Registered Nurse (RN) Home Health for a nursing job in Atlanta, Georgia. Job Description & Requirements Specialty : Home Health Discipline : RN Duration : Ongoing 36 hours per week Shift : 12 hours, days Employment Type : Staff Overview : The RN Case Manager provides and supervises comprehensive home health care to 25–35 patients. The role involves planning, coordinating, and evaluating patient care throughout the episode—from admission to discharge—ensuring all physical, emotional, and clinical needs are met. The RN leads interdisciplinary collaboration with physicians, therapists, social workers, LPNs, aides, and families. Key Responsibilities Direct Patient Care : Conduct in-home assessments, create and adjust nursing care plans, and perform skilled nursing procedures per physician orders. Care Coordination : Oversee all disciplines involved in care; supervise LPNs and Home Health Aides; ensure timely recertifications and discharge planning. Patient & Caregiver Education : Teach disease management and promote self-care to support recovery and independence. Communication : Report status updates, changes, and lab results to physicians; participate in care team meetings; maintain regular communication. Regulatory Compliance : Complete documentation accurately in EMR, including OASIS; follow CMS, State, and Joint Commission guidelines. Professional Development : Support orientation and education of new staff; attend continuing education sessions; maintain current clinical knowledge. Performance Improvement : Engage in departmental quality initiatives and system-wide committees to enhance patient outcomes. Other Duties : Meet productivity goals; participate in weekend / call rotations; comply with WellStar policies and code of conduct. Requirements Education : Graduate of an accredited nursing program. Licensure / Certifications : Active RN license (Single State or Compact). Basic Life Support (BLS) required. Experience : Minimum 1 year in Medical-Surgical nursing required. Home Health and ICU experience preferred. Skills : Strong critical care assessment, leadership, problem-solving, and communication skills. Proficient in EMR documentation, OASIS (preferred), and home health operations.
