Washington State RN Care Manager

A company is looking for an RN Care Manager.

Key Responsibilities

Improve health outcomes and reduce care costs for children through care management

Implement transitional care, ED visit follow-up, and care coordination strategies

Educate staff and families on care coordination processes and track patient outcomes

Required Qualifications, Training, and Education

BSN degree or equivalent combination of education and experience

Minimum of five (5) years of recent clinical experience

Experience in community-clinic settings and case management for complex children and families

Current Washington State Nursing License and Drivers License

Current BLS for Healthcare Providers certification
