Position Summary :
The Community Health Worker is responsible for providing comprehensive, client-centered case management services to eligible children (birth through age 20) and pregnant women enrolled in Texas Medicaid who have a health condition, health risk, or high-risk pregnancy. This role focuses on ensuring clients receive access to essential medical, social, educational, and other necessary services through coordination, advocacy, and resource linkage. Must live in / near and willing to travel to Conroe, Willis, The Woodlands, New Caney, Magnolia, and Pinehurst.
Key Responsibilities :
Client Assessment & Engagement
- Conduct thorough needs and strengths assessments for eligible clients.
- Complete and maintain required documentation, including the Family Needs Assessment and Service Plan.
- Develop individualized, culturally sensitive service plans aligned with client needs and preferences.
- Confirm voluntary participation and obtain informed consent for case management services.
Service Coordination & Linkage
Assist clients in accessing medical services such as pediatric care, OB / GYNs, dental care, and therapy.Facilitate access to social services including Medicaid transportation, housing assistance, and food programs.Coordinate educational support services including ARD meetings for special education and school-based accommodations.Collaborate with Managed Care Organizations (MCOs) and school personnel to ensure cohesive care planning.Advocacy & Empowerment
Serve as a liaison between families and providers to advocate for services and eliminate barriers to care.Educate families on navigating the healthcare system, public programs, and local resources.Promote family engagement and self-advocacy through coaching and support.Compliance & Documentation
Ensure all services meet Medicaid documentation, billing, and compliance requirements.Maintain timely, complete, and accurate records of all services delivered.Monitor client progress, follow up consistently, and adjust service plans as needed.Program Participation
Attend ongoing training related to Texas Health Steps, Medicaid policy, and CPW updates.Participate in internal audits, quality improvement processes, and team meetings.Qualifications :
Must have a bachelor's degree or minimum five years of case management experienceMust hold one of the following credentials :Certified Community Health Worker (CHW) or
Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW) under supervision, Licensed Registered Nurse (RN), Certified Doula (in alignment with CPW guidelines)Must be Medicaid-enrolled or eligible to enroll as a provider.Experience working with maternal, child health, or medically vulnerable populations.Knowledge of community-based services, health disparities, and social determinants of health.Preferred :
Bilingual (English / Spanish).Prior experience with Medicaid billing and documentation.Familiarity with Texas Health Steps and / or CPW policies and procedures.Skills & Competencies :

Flexible schedule; some home visits or virtual meetings may be required.Travel may be required to client homes, schools, or service provider locations.Must maintain client confidentiality in compliance with HIPAA.
