Full-time Outpatient Palliative Care Nurse Practitioner Opportunity available in Northwest Ohio
Hospital employed position
Services including home visits, extended care facility consultations, telehealth, and clinic-based care.
Manage 6 8 patients per day based on location across a 9-county area in NW Ohio.
Hours are Monday-Friday 8a-5p, with no call or weekends
Participate in delicate, goals-of-care discussions with patients and families facing serious or terminal illnesses.
Assess, plan, implement, and evaluate individualized care needs in alignment with palliative care principles.
Collaborate with a supportive interdisciplinary team including other NPs, physicians, and administrative staff.
Participate in care transitions, including determining when patients would benefit from continued palliative care or hospice services.
Work closely with a hospitalist team responsible for all patient admissions.
Document patient encounters thoroughly and maintain excellent communication with patients, families, and care teams.
