Shift : MondayFriday days with potential weekend rotation and on call

Job Description :

Sunbelt Staffing is seeking a compassionate Hospice Home Health RN to support patients and families in Auburn, ME. This role involves delivering quality end-of-life care, managing patient needs in the home setting, and providing guidance to families during a critical time. Nurses should be comfortable with independent work, weekend rotation, and on call responsibilities.

Key Responsibilities :

Provide direct hospice nursing care in home settings

Develop and manage individualized care plans

Coordinate with interdisciplinary team members

Offer patient and family education and emotional support

Ensure accurate charting in Homecare Homebase

Qualifications :

Active RN license in ME

Minimum 2 years of nursing experience

At least 1 year of hospice home health experience required

BLS certification

Proficiency with Homecare Homebase

Valid drivers license, reliable transportation, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
