Excite Health Partners is seeking a travel nurse RN Case Management for a travel nursing job in Salinas, California.

Minimum of 5 years experience working in Case Management setting. Would like a “Generalist” candidate who has Case Management experience, as well as Discharge Planning (for all disease states) and Utilization Review. Previous travel experience is highly preferred. MUST HAVE experience with InterQual software; Meditech experience is preferred.

Must be a team player and work with Physician to state whether or not the patient meets criteria for admission. If they do not meet criteria, what are the other options for the patient. Candidate must have confidence in speaking with Physicians and understand disease trajectory. Performs utilization reviews with payers; works with Acute Care Management Nurse, bedside Staff Nurse, and Social Worker for clinical coordination of patients’ hospital stays while complying with CMS rules and regulations and conditions of participation, as well as commercial payer-specific guidelines to prevent denials and revenue loss. Assignment is based on the following service lines : ED, Medicine Team, Family Practice, Trauma, and “assignment A” (which consists of ICU, Surgery, MIU, L&D, NICU, Pediatrics) Case load between 15-20 (adult inpatient); A assignment varies (minimal discharge planning need for the services lines under A). ED varies.

Requirements : Registered Nurse in the State of California and BLS issued through American Heart Association. CCM / ACM preferred, but not required.

Will have weekend and holiday rotation. Will be working with other CMs assigned to different units. Our goal is to have two inpatient CMs during the weekend; currently, we have one nurse assigned to the inpatient units. Will need to be flexible to work on other teams. They do like consistency; however, that is not always possible depending on staffing and peak visit times.

Dress code : Business casual or any color of professional scrubs. No other facility logos permitted.

Orientation : Will accommodate start any day of the week. Will start in Human Resources and complete paperwork and obtain badge, next onto Occupational Health for Respiratory Fit Test and then will meet with Nursing Education to complete appropriate testing for their unit. All RNs will take a medication test the first day. After testing, will go to the unit to finish their shift with a Clinical Preceptor. Employee will be provided with a one week unit specific training to familiarize with the documentation, processes, and resources.
