Trinity Health Of New England seeks a  Nurse Practitioner or Physician Assistant  to join the Cardiology Heart Failure  team at Saint Francis Hospital and Medical Center.

RESPONSIBILITIES :

  • Seeing patients admitted with heart failure in consultation with the cardiology team
  • Performing advanced heart failure consultations
  • Working with the interdisciplinary team made up of hospitalists, social workers, nurses, Cardiologists
  • Working as an extension of the physician in managing patients with heart failure.
  • Skillset includes : Management of diuretics, Guideline directed medical therapy initiation, Management of cardiogenic shock
  • Attending interdisciplinary rounds daily

The successful candidate will :

  • Be interested in heart failure care
  • Have Cardiology / Heart Failure experience (preferred but not required)
  • Hold an Advanced Practice Nurse or Physician Assistant CT state license.
