You'll drive preventative care, empower individuals to achieve personal health goals, and ensure every person receives the best possible support to live their healthiest life.\n\nWho will love this job : \n A thoughtful mediator- you're receptive and remain authentic in all situations\n A collaborative teammate - you love working with others and rely on the strong relationships you build to achieve the best outcomes for the people you serve\n A natural advocate- you offer reassurance and support throughout day-to-day activities as the liaison between the people we serve and their care teams.\n A helper - you can easily mend fences and have a knack for winning hearts and minds.\n\nWhat YOU'LL will do : \n Provide nursing support and consultation with the state's Nurse Practice Act and physician orders.\n Ensure individuals receive routine and preventative health services and medication administration is completed accurately according to regulations.\n Provide health related education to those served and those who serve in the agency.\n Maintain and review electronic healthcare records to monitor pertinent health status, provide accurate and through documentation and update agency leaders as needed\n Build effective relationships with colleagues, persons served and their providers and families / guardians.\n\nSCHEDULE : Days, 8 hour shifts, can work up to 24 hours a week - required to work at least 24 hours a quarter, NO HOLIDAY, ONCALL or NO WEEKEND hours requirements, travel within Grand Island, Hastings, York, Axtell may be required - agency vehicle provided.\n\nCommitment to Inclusion, Diversity, Equity and Belonging : \n\nAt Mosaic, we believe in creating a workplace where everyone has the chance to contribute and succeed.
