Description :

The case manager will work in an inpatient acute rehabilitation setting.

The position requires managing patient care from admission to discharge.

It includes conducting initial assessments and psychosocial evaluations.

Crisis management is a crucial aspect of this role.

The case manager will facilitate interdisciplinary team meetings and family meetings.

Responsibilities include arranging follow-up services and coordinating care.

Experience in acute hospital settings is preferred.

Familiarity with electronic medical systems, such as Medconnect and Cerner, is advantageous.

The role involves discharge planning, benefits eligibility verification, and care coordination.

Candidates must be eligible for social work licensure if applicable.

The position requires flexibility with weekend and holiday coverage as needed.

Business casual or black scrubs are the accepted dress code.

All employees must receive the flu vaccine, with no exemptions allowed.

First-time travelers are welcome, provided they have relevant experience.

The case manager will work primarily on an orthopedic and amputee unit.
