Palliative Care Coordinator - Advanced Illness Management Team

Overview :

We are looking for a Care Coordinator for our AIM (Advanced Illness Management) to join our team in our Addison, TX palliative care branch. This position will directly report to the Director of AIM and is responsible for managing all day to day business affairs and other operational activities related to the administration of the advanced illness management (AIM) and palliative care department by performing the following duties personally, including intake function, billing function, medical record maintenance function, and marketing function.

• This is a non-clinical position.
• Experience with referral, intake, scheduling in a home care setting, hospice / palliative a plus, and will be primarily assigned to Addison, TX patients, but may work across several branches within Illumia.
• Position is located in Addison, TX palliative care office.
• Bilingual Spanish preferred
