What You Should Know About the LPN / RN Extended Hours - Individualized Care Role : \n

• This is a Part-Time DRIVING position covering Newark and surrounding areas\n
• Hours are 4p-12a one to two days a week, and 8a-8p every other weekend. Also include holiday requirements.\n

Only those who have a heart for hospice will succeed\n\nThe LPN / RN Individualized Care Essential Duties Are : \n\nWe provide superior care for patients in families in homes / facilities. The LPN / RN will provide quality nursing care to patients in a manner that meets the patient and family needs, working under the direction and supervision of an RN. You will provide superior care to patients and their families.\n\nLPN / RN Individualized Care Qualifications : \n
• Registered LPN or RN with the State of Ohio without any board actions.\n
• One year of nursing experience preferred.\n
• CPR Certified.\n
• Computer skills sufficient to properly document services and care.\n
• Ability to drive during daytime, nighttime or inclement weather.\n
• Valid Driver's License with Safe Driving Record.\n
• State Minimum Automobile Insurance Coverage.\n
• Must be able to pass a criminal background check\n
