Location : 19 Warehouse Row, Albany, NY
No Weekends or Holidays
The St. Peter's ALS Center serves patients and families in the greater Capital Region, providing crucial support, services, and coordination. We have a rewarding opportunity for a part-time RN Care Manager to join our team and manage a caseload of patients and families.

What you will do :
Manage a caseload of ALSpatients and families from diagnosis through the course of disease.
See patients at ALS Center clinic
Work with an interdisciplinary team of ALS experts.
Responsibilities :
The RN is responsible for assessing the needs of each patient and working with the team to implement a plan of care.
Make occasional home visits to patients / families when they can no longer attend the ALS clinic, as needed.
What you will need :
New York State RN License.
Associates Degree in Nursing. BSN Preferred
2 years of experience preferred (in case management, hospice, rehab, home health, and other relevant experience)
Experience and desire to work as part of an interdisciplinary team
Excellent problem solving, organization, and multitasking skills preferred
Pay Range : $36.50-$49.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
