Job Summary :
Home & Community Based Services is a dynamic program that enables older adults and disabled populations to live fully integrated lives within their own communities. Social Work Case Managers and Registered Nurses / LPNs work closely with members using a strengths-based model to design individualized service plans.
Essential Duties and Responsibilities :
Develops person centered service plans and completes periodic reassessment at least twice per year to determine service needs, including activities that focus on identification of any medical, educational, social or other services.
Arranges and coordinates services proposed in person centered service plans in accordance with accepted standards.
Prepares cost estimates for care plans in compliance with waiver requirements.
Makes home visits and / or telephone contact as needed to evaluate service effectiveness and assess continuing member needs.
Monitors the quantity, quality and cost of services provided under each care plan.
Maintains compliance with the Department of Public Health and Human Services (DPHHS) and agency requirements for documentation of care.
Assists in service coordination to ensure members health and safety and addressing service and provider issues.
Qualifications :
Bachelors degree in Social Work or related behavioral science degree from an accredited college, a valid drivers license, and appropriate auto insurance and reliable transportation. One (1) year of previous experience in human services and / or experience with the elderly and disabled populations. Working knowledge of community resources preferred. High speed Internet required for remote work.
This is a work-from-home position that requires residency in one of the counties contiguous to Missoula County, Montana. Occasional in-office work required. Must be willing to travel throughout the program service area (Ravalli, Mineral and Missoula Counties).
