Position Overview HumanEdge Health is seeking a skilled Physical Therapist (PT) for a 13-week travel assignment in Mound City, Missouri. The PT will provide evaluation, treatment, and rehabilitation services to patients with physical functioning disorders and / or wound care in a long-term care or rehab setting.

Responsibilities Screen, evaluate, and rehabilitate patients with physical functioning disorders and / or wound care

Develop and implement individualized treatment plans following regulatory and clinical practice standards

Utilize standardized assessments and evidence-based practices to guide clinical interventions

Administer therapeutic procedures including manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, wound care, etc.

Assess patient progress and adjust treatments accordingly in the clinical record

Evaluate treatment outcomes for effectiveness and modify services as needed

Provide consultation and counseling to patients, families, caregivers, and other service providers

Communicate effectively with supervisors and interdisciplinary team members regarding patient care

Prepare comprehensive discharge summaries and maintain accurate patient records

Accurately document treatment minutes per regulatory and clinical requirements

Participate in patient care conferences, utilization review meetings, and rehabilitation meetings

Attend facility and company-required trainings, in-services, and continuing education

Obtain necessary DME or supplies per regulatory and clinical practice requirements

Supervise and support physical therapist assistants, rehabilitation aides, and / or students

Adhere to APTA and state-required practice acts, codes of conduct, and ethical standards

Submission Requirements Minimum 1 year PT experience – Required

LTC or Rehab experience – Required

MO State License – Required

New grads accepted with Level II fieldwork experience
