Licensed Practical Nurse (LPN) - Transitional Care Unit Magee

The 20-bed Transitional Care Unit serves primarily short-stay recuperative and rehabilitation patients—a majority are geriatric and / or stable post-op orthopedic and oncology patients. The TCU is a family- and patient-focused environment where collaboration and communication are paramount, and you should be comfortable working with wound vacs, hoyer lifts, and standard IV therapy. You will partner with the patient and his or her family, along with a diverse roster of therapists and counselors to help the patient return home with as much independence as possible. The TCU features a wide array of recreational activities, 24-hour visitation for family and friends, and an onsite PT / OT gym. OAPSA clearance is required.

What can you Expect as a Licensed Practical Nurse?

The LPN is prepared to function as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency. The LPN participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place. An LPN shall obtain instruction and supervision if implementing new or unfamiliar nursing practices or procedures. An LPN shall follow the written, established policies and procedures of UPMC, UPMC LPN Practice Guidelines and the State Practice Act. Acts within the LPN Practice guidelines (except IVs) and within the framework of Relationship Based Care to create a healing environment.

An LPN shall communicate with a licensed professional nurse and the patients’ health care team members to seek guidance when :

• The patients care needs exceed the licensed practical nursing scope of practice.
• The patients care needs surpass the LPN’s knowledge, skill, or ability.
• The patient’s condition deteriorates or there is a significant change in condition, the patient is not responding to therapy, the patient becomes unstable, or the patient needs immediate assistance.
