This mission is accomplished through a unique blend of prevention, placement, and housing programs all designed to help elders find and succeed in homes of their own. Overview :
The Adult Clinical Community Services (ACCS) Licensed Clinician at Hearth plays a key role in delivering direct clinical services. This position is responsible for clinical case management, treatment planning, crisis intervention, and interdisciplinary coordination, while ensuring compliance with DMH regulations and Medicaid Rehab Option standards.
$3,000 Sign-On Bonus – paid in two installments ($1,500 after 3 months and $1,500 after 6 months of successful employment).
Location : Hearth’s Supportive Housing facility at 1640 Washington street and other sites.
Essential Functions :
Clinical Service Delivery & Documentation
- Conduct intakes, screenings, and bio-psychosocial evaluations, assessing mental health, physical health, social support systems, and family relationships.
- Develop, implement, and update Medicaid Rehab Option-compliant treatment plans, ensuring all plans align with DMH requirements and best clinical practices.
- Document all clinical activities in both CX360 (state system per DMH requirements) and Hearth’s internal tracking system, ensuring accuracy, compliance, and continuity of care.
- Collaborate with an interdisciplinary team, including nursing, social care, program directors, and resource specialists, to ensure a coordinated and holistic approach to care.
- Provide direct clinical interventions, including individual counseling, skills training, and support in community living environments (SIE).
- Participate in monthly interdisciplinary meetings, reviewing treatment plans, progress, and service effectiveness.
Case Management & Community Support
Provide clinical case management, ensuring timely referrals and coordination with community providers for mental health, substance use, legal, financial, and social services.Assist clients with public benefits navigation, including Medicaid, Medicare, SSI, SSDI, and food stamps, ensuring continued access to essential resources.Act as a liaison between Hearth, DMH, and external service providers, ensuring seamless care transitions and integrated services.Promote self-sufficiency, engagement in treatment, and successful community tenure through targeted interventions.Crisis Management & Risk Prevention :
Lead crisis response efforts, ensuring swift and appropriate interventions for PS experiencing acute distress.Develop, update, and implement Safety Plans and Crisis Prevention Plans, ensuring risk mitigation strategies are in place for all high-risk cases.Collaborate with emergency services, DMH risk management teams, and legal representatives to provide real-time support and crisis resolution.Ensure proactive engagement with high-risk PS, working to prevent hospitalizations, homelessness, and service disengagement.Support staff in managing crisis situations, ensuring effective de-escalation strategies and appropriate follow-up care.Additional Responsibilities :
Participate in on-call rotations for nights and weekends, providing clinical coverage and emergency support.Attend monthly interdisciplinary meetings to discuss client progress, treatment effectiveness, and programmatic improvements.Qualifications & Requirements :
Education & Licensure
Master’s degree in Social Work, Psychology, Mental Health Counseling, or a related field.Licensure : LCSW / LICSW / LMHC. Candidates with LMFT, BCBA, LADC 1, RN, or Licensed Psychologist (in a mental health-related field) will be considered.Experience & Skills
2+ years of clinical experience working with individuals experiencing psychiatric disabilities, co-occurring disorders, and housing instability.Strong knowledge of evidence-based practices, including Motivational Interviewing, Harm Reduction, Cognitive Behavioral Therapy (CBT), and Seeking Safety.Experience in risk assessment, crisis intervention, and care coordination with external agencies.Knowledge of CX360 preferred, DMH regulations, and Medicaid Rehab Option documentation preferred.Certifications & Additional Requirements
CPR and First Aid certification (must obtain within two weeks of hire).Must be at least 21 years old and pass a multi-state background check, reference check, and licensure verification.Valid driver’s license and reliable transportation required for community-based work.Equal Opportunity Employer :
