The Nurse Care Manager in DD Healthcare Home is part of the person-centered integrated care team and provides individual and population-based health interventions to persons enrolled in the DD Health Care Home by integrating care for physical health conditions.

This is a hybrid position with community travel that will cover St Louis County / City and St Charles County areas to provide health assessments in-person at client homes. Monday-Friday, daytime hours. No overnights / weekends.
