Missouri and Kansas Licensed Clinical Nurse

A company is looking for a Clinical Nurse, Case and Disease Management.

Key Responsibilities

Conducts member assessments and develops individualized care plans

Monitors and evaluates the effectiveness of care plans, utilizing various healthcare resources

Acts as a liaison between members and healthcare providers to ensure coordinated care

Required Qualifications

Associate degree in nursing

3 years of full-time direct patient care clinical experience in transplant, oncology, or medical / surgical

Valid and active Registered Nurse (RN) license in Missouri and Kansas

Knowledge of URAC and NCQA guidelines, as well as state and federal regulations

Case Management Certification within 3 years of hire or transfer to the role
