Primary duties include :
Assisting with assessment, planning, implementation, and evaluation of patient care based on age and diagnosis
Effectively managing patient care in coordination with a multidisciplinary team
Communicating information appropriately according to agency policies and procedures
Contributing to program effectiveness
Organizing and performing work efficiently
Maintaining and adjusting schedules to enhance agency performance
Demonstrating commitment to agency values
Developing positive interpersonal relations
Maintaining customer satisfaction
Demonstrating the agency's mission, vision, and values daily
Responsibilities may include :
Providing direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
Documenting patient information accurately and thoroughly in accordance with agency policies and procedures
Collaborating with interdisciplinary teams to develop and implement individualized patient care plans
Participating in quality improvement initiatives to improve patient outcomes and agency performance
Requirements include :
Graduate of an accredited or approved school of practical nursing
Current license to practice practical nursing in North Carolina
CPR certification
Minimum one year practical nursing experience, preferably in home health
Ability to transfer and maneuver objects weighing 50 pounds
Ability to communicate and be literate in English
Physical abilities required for this position include :
Transferring patients safely and efficiently
Administering injections and medications as prescribed by physicians
Manipulating equipment and medical devices as necessary
Additionally, mental abilities required for this position include :
Assessing patients' physical and emotional needs
Formulating plans to meet those needs
Explaining treatments and procedures to patients and families
Sensory requirements include :
Visual assessment of patients and surroundings
Auditory skills for auscultation and communication
