Job Title : Physical Therapist – Home Health

Location : Berkshire Health System – Massachusetts

Contract Type : Travel Assignment

Job Summary

The Physical Therapist is responsible for providing home health physical therapy services to patients under the guidance of a comprehensive plan of care. This includes direct patient care, evaluation, and collaboration with other healthcare professionals to achieve optimal patient outcomes. The role also involves patient education, equipment recommendations, and ensuring adherence to best practices in home health care.

Key Responsibilities

Deliver physical therapy services to patients in the home health setting, focusing on rehabilitation and improved mobility.

Perform patient evaluations, develop treatment plans, and implement therapy based on medical necessity.

Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care.

Train patients and families on the safe use of rehabilitation equipment (splints, orthoses, prostheses, wheelchairs, ADL equipment, and other modalities).

Complete OASIS documentation accurately and in compliance with regulatory requirements.

Provide education to patients and caregivers on therapy techniques and home exercise programs.

Monitor patient progress and adjust care plans accordingly.

Maintain compliance with agency policies, state regulations, and home health standards.

Participate in interdisciplinary meetings to review patient progress and update care plans as needed.

Job Requirements

Qualifications & Requirements

Education : Graduate of an accredited Physical Therapy program.

Experience :

Minimum 2+ years of acute nursing care (hospital-based) and home health experience – Required

Certified home health experience with proficiency in OASIS – Non-Negotiable

Acute care experience – Preferred

Licensure / Certification :

Valid Massachusetts Physical Therapy license – Required

Current AHA BLS Certification – Required

Other Requirements :

Valid Driver’s License – Required

Must be able to work independently with minimal supervision.

Strong communication, organizational, and problem-solving skills.
