Case Manager, Homeless Initiative Program (HIP)

Summary : Monitoring service delivery of caseload

Position Type : Full Time, Non-Exempt

Essential Functions

  • Review referrals and assess program eligibility
  • Complete intake and initial assessment of services needed
  • Develop and monitor service plans in accordance with regulatory guidelines
  • Assure that units are leased within budget guidelines
  • Maintain contact with program individuals in accordance with regulatory guidelines
  • Assist with developing and maintaining monthly budgets, where applicable, for program individuals
  • Assist program individuals in obtaining community support, resources and services
  • Maintain professional, courteous relationships with all consumers, staff, community members and professional partnerships
  • Assist Program Director with APR report
  • Assist with budget monitoring and planning
  • Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time cards, requesting edit sheets, etc).
  • Report any significant concerns to supervisor
  • Maintain training requirements
  • Projects and duties as assigned

    Required Education and Experience

    Bachelor’s degree in Human Service field. Good computer communication, interpersonal, and writing skills required. Ability to work independently.

    Physical Demands

    While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and / or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
