CNA - 1st shift (7a-3p) Home Care

JOB TITLE : CNA

DEPARTMENT : Home Care Agency

REPORTS TO : Home Care Director

SHIFT : 1st shift 7a - 3p, including working every other weekend

CLASSIFICATION : Non-Exempt

POSITION SUMMARY :

Responsible for assisting Home Care Agency clients with personal care and homemaker needs.

Essential Duties and Responsibilities :

• Use appropriate skills and abilities to support Home Care Agency clients with personal care needs.
• Review and follow current care plan developed for the client, and appropriately document care provided.
• Communicate with supervisor and Home Care Agency scheduler of availability to work specific days / hours, and provide timely requests for time off or other changes in availability.
• Attend departmental meetings and trainings, and comply with requirements for Relias Online Learning courses.
• Inform supervisor and nurses in the appropriate level of care of any changes or concerns with client functioning, health, and overall well-being.
