Social Worker

Directly provides counseling, case management and other chemical dependency services in meeting the social emotional needs of the detox / rehab patients and their family members.

Responsibilities :

• Provide assigned patients with activities, group sessions to plan objectives, stimulate behavioral change and to process effectively.
• Provide evaluations and psychosocial assessments for patients in collaboration with the td team.
• Initiate, develop, coordinate and evaluate treatment plans in providing appropriate treatment for individual patients.
• Prepares and keeps a current case record for each assigned patient according to HealthAlliance Hospital, OASAS, and JC standards.
• Participate as a team member in conferences, treatment planning on behalf of patient.
• Attend in-service training and keeps professionally updated through reading and attending seminars.
• Provide lectures, workshop / training for patients and staff as assigned.
• Strictly adheres to all of the confidentiality requirements imposed by law and professional conduct.
• Observes adherence to current policies and a resource to director for program recommendations.
• Perform any other duties as assigned.
